The Chronic Disease Management Tools Course (HLTADM010) is a nationally accredited unit of competency that aims to provide students with the skills and knowledge required to provide instructions and information to clients, proactively follow up cwith lients and liaise with other service providers to support coordinated care approach.
The Chronic Disease Management Tools Course (HLTADM010) aims to provide students with knowledge and skills to develop an understanding of the chronic disease legal framework, develop an awareness of the chronic disease management tools such as GPMPs, TCAs, and Allied Health visits, perform the elements of assessments and manage client’s utilising care planning and funding models that provide the best outcomes for both the client and health care service.
The course is supported by funding from the Australian Government Department of Health under the Chronic Disease Support Project.

This course is designed to support Aboriginal Health Workers and Practitioners, Practice Nurses and others working in primary health care, whose work involves Care Planning for clients with chronic diseases and want to increase their knowledge in the tools available to assist in this work.
Each subject builds on to the next, with assessments at the end of each topic.
Upon successful completion of the Chronic Disease Management Tools course students should have the ability to:
- Support and understand the needs of Aboriginal and Torres Strait Islander patients
- Understand the legal frameworks underpinning chronic disease management
- Understand funding models in primary healthcare
- Understand the essential elements of health assessments
- Understand the importance of data to inform health care.
Online learning with a mix of multichoice quizzed and assessments marked by the Benchmarque Group quallified assessors.
It is recommended students review the Foundations of Chronic Disease in Primary Healthcare Course prior to attendance at the workshop.
To achieve competency students must demonstrate successful completion of:
- All assessments in a mix of multichoice quizzes and long answer assessments marked by the Benchmarque Group quallified assessors.
Build and strengthen competency for proactive patient care in chronic disease management, including patient education, and the coordinated care requirements from allied health professionals.
